Presenting complaint: Follow-up for hypertension management
Examination: BP 135/85 — improved from 142/90 at last visit. HR 72 regular. No peripheral oedema.
Assessment: Hypertension — improving on current regimen
Plan: Continue Amlodipine 5mg daily. Reinforce low-sodium diet. Review in 6 weeks. Request U&E bloods.
Presenting complaint: 3-day history of productive cough and low-grade fever
Examination: Temp 37.4C. Throat mildly erythematous. Chest clear bilaterally. No lymphadenopathy.
Assessment: Viral upper respiratory tract infection
Plan: Symptomatic Rx — paracetamol, throat lozenges, increase fluids. Return if symptoms worsen or persist beyond 5 days.
Presenting complaint: Well-child visit — 18-month developmental check
Examination: Weight 11.2kg (50th centile). Height 82cm (50th centile). Development appropriate for age. Fontanelle closed.
Assessment: Normal growth and development at 18 months
Plan: 18-month vaccinations administered (DTaP-IPV booster). Next well-child check at 2 years. Advised re: nutrition, developmental milestones.
